About the Author

Fred Busch is a Training and Supervising Analyst at the Boston Psychoanalytic Society and Institute, a Geographical Supervising Analyst of the Minnesota Psychoanalytic Institute, and a Visiting Supervisor at the Vermont Psychoanalytic Institute.Dr. Busch has published over 70 articles in psychoanalytic literature, and three books, primarilty on the method and theory of treatment. His work has been translated into seven languages, and he has been invited to present over 160 papers and clinical workshops nationally and internationally.

Clinically oriented update of psychoanalytic theory

Fred Busch is a practicing psychoanalyst influenced by the 'ego psychology' tradition of American psychoanalysis. Among those circles he is well known for his patient-centered clinical theory that focuses on patient capacities for learning and/or resistances to self-knowledge (what his colleagues would call defense mechanisms and character analysis). He advocates a psychotherapy based upon free association and symbolization in the 'here and now' rather than upon 'lifting unconscious repressions' originating from childhood traumas. His latest book is Creating A Psychoanalytic Mind (Routledge, 2014). In my reading of it, his main aim is to update psychoanalytic theory to match the evolving/improving clinical practice of psychodynamic psychotherapy.

Psychotherapy, ideally for Dr. Busch, is about cultivating the patient's abilities to learn about the self. Psychoanalysis should train patients in how to adopt a 'psychoanalytic mind' for themselves. This does not require remembering all the facts of one's childhood dramas. Rather the goal is enhancing capacities for self-observation, to be able to think about how one tends to think: why I am ruminating on this conversation from last night (over the content of what was said)? what does this one person from the past plague my dreams (over facts about the person)? what made me recall this memory in this moment (over the details of the specific memory)?

Busch calls this Process Knowledge, which he distinguishes from contents or states of knowledge. Process knowledge contributes to ego formation as a vantage point from which to view one's habitual action-tendencies. Action and emotion without thinking is the problem for Busch. He claims that a commonality running through many mental disorders is the inability to verbalize one's fears, traumas, and/or rages (Freud calling these 'thing presentations' in the Interpretation of Dreams, as opposed to more meaningful 'word presentations'). One one hand, such symbolic deficiencies produce psychosomatic symptoms, mainly, compulsive ego behavior. On the other hand, this does not mean one can get better simply by obtaining a diagnosis or gaining intellectual knowledge about one's condition, or from any purely intellectualizing attitude. Getting better through ego symbolization is slow and steady work (and that is how Dr. Busch appears to work with most of his patients, i.e. over years several times a week).

This book grapples with a therapeutic paradox: how can the patient, or any person for that matter, come to see their own limitations, their unique but constraining ways of behaving, the as yet unrecognized contours of their character? how can one help a narcissistic person understand how their narcissism interferes with their relationships? or an anxious person how needless their small worries are? or an obsessed person how excessive their rumination?

Busch disentangles the dilemma into two parts: 1. The analyst discovers the patient's inner character conflicts through 'language action,' that is, speech that is enactive in purpose rather than communicative. Language action is speech that has not fully internalized or cogitated its sensorimotor roots. It is usually more concrete, disjointed, rehearsed, and/or performative than symbolic thought. It is speaking that provokes rather than describes, indicating some psychic regression. Such speech is detected through the 'countertransferential feelings' of the analyst. 2. Then the analyst joins the patient in a stroll through the latter's local psychic neighborhood (the Preconscious), asking them discerning questions about the local lore. The technique should serve to wake up the thinker, as Busch puts it, to enable the ego to view its own defense. He writes, "Our method relies on trying to engage the patient's higher-level ego functions to deal with the most regressive parts of the personality" (2014: 51).

I'm only scratching the surface. Much of the book reads as a polemic against conventional 'topographic' psychoanalysis for obsessing over the unconscious, past traumas, and 'premature' interpretations of transferential Oedipal feelings. Busch's practice and proposals indeed seem much more humane, more patient-centered, and if not about happiness per se, certainly about improving the patient's self-knowledge.

They express them in ways relevant to urban planners, cultural planners and civic leaders of all stripes. The book in itself crosses discipline borders and begins a discussion on urban placemaking that is long overdue. This should be required reading in urban planning, cultural studies and municipal management, among others.
